Faith Based Wellness Program

  • This Community Health Services program is designed to assess and address the health needs of members of faith-based communities. It has been demonstrated in evidence-based studies that faith-based wellness programs prove to be very successful in improving health because members hold great trust in their faith and spiritual leadership. 

  • The Community Health Services Department works closely with members of faith based communities to coordinate health fairs at  faith-based centers. Members of the department provide Health fairs consisting of various health screening services and education. Registered nurses and advanced nurse practitioners also provide follow-up services to participants.

Services provided at fairs can include:

  • Blood sugar screenings
  • Blood pressure screenings
  • Cholesterol screenings
  • Osteoporosis screenings
  • Vision screenings
  • Audio screenings
  • DermaScan facial screenings
  • Immunizations
  • Referrals for community healthcare services
  • Follow-up health care services
  • Health Education;
  • Health education encompasses the following areas:
  • Cancer Education: General Prevention and Risks
    • Prostate health
    • Skin cancer
    • Breast health
  • Cardiovascular Disease
  • Diabetes
  • First Aid
  • Health careers/van tours
  • Healthy nutrition
  • Hypertension
  • Osteoporosis
  • Physical fitness
  • Stroke awareness
  • Sun safety
  • Falls prevention
  • Ask the doctor and/or APN
  • Ask the pharmacist
